I used to use a keyboards with a built-in trackball, which sat comfortably
on my lap. It was so effective my boss got one for himself, and then his
boss got one...

Speaking of balls, I find that Chinese iron balls [1] regularly help with
RSI. A health and safety officer at a former employer was an advocate of
using them, and had a lovely collection of them on her desk.

[Should I expect subthread of puns about playing with my balls....?]

Well, it gives me something to do with my hands when I'm thinking. (You stop
to think while programming, right?)

Off Topic: Learning or adding keyboard shortcuts in place of using the mouse
will do wonders.
